The Second Leading Cause of Lung Cancer — And Most People Have Never Heard of It

There is something genuinely strange about the public awareness landscape around radon. Here is a substance responsible for approximately 21,000 lung cancer deaths in the United States every year — more than drunk driving, more than falls in the home, more than house fires — and the majority of Americans cannot tell you what it is, where it comes from, or whether their home has been tested for it. The gap between the scale of the problem and the level of public awareness is one of the more puzzling public health disconnects of the past several decades, and understanding how it developed helps explain why the problem persists and what closing the gap actually requires.


This is not a post about the science of radon — that is covered in our foundational post on what radon is and why Michigan homeowners should care. This is a post about the awareness gap itself: why it exists, what sustains it, and why the story of radon's impact deserves to be told more clearly and more often than it currently is.


The Numbers That Should Have Changed Everything


When the scale of radon-related lung cancer mortality became clear in the 1980s and 1990s, there was genuine expectation in public health circles that it would generate the kind of sustained national response that other major health risks had produced. The 1984 discovery of Stanley Watras — a nuclear plant worker in Pennsylvania whose home radon levels were so extraordinarily high that he was triggering radiation monitors at his workplace — generated significant media attention and prompted the EPA to establish residential radon guidelines. For a moment, it seemed like radon was about to enter the mainstream of American health consciousness in a lasting way.


It did not. The initial burst of attention faded. Radon testing rates remained low. The regulatory response — voluntary rather than mandatory, awareness-based rather than enforcement-driven — did not produce the widespread behavioral change that public health advocates had hoped for. And year after year, the death toll continued: approximately 21,000 Americans per year, every year, from a cause that is both well understood and largely preventable.


To put that number in perspective: lung cancer from radon kills more Americans annually than the total deaths from breast cancer in women, more than all drunk driving fatalities, and more than all drowning deaths combined. It is not a minor or obscure health risk — it is a major cause of preventable death that has somehow failed to claim the public attention it statistically deserves.


Why Radon Has Stayed Under the Radar


Several factors have combined to keep radon awareness lower than the mortality numbers warrant, and understanding them helps explain why this particular health risk has been so difficult to elevate in public consciousness.


No acute symptoms. The most powerful driver of health awareness is immediate, visible harm. Carbon monoxide poisoning produces dramatic, obvious symptoms and can kill within hours. Heart attack symptoms are unmistakable and urgent. Food poisoning announces itself forcefully. Radon produces no symptoms — not immediately, not after a month of exposure, not after a year. Its harm accumulates silently over decades and manifests as lung cancer that may not develop for twenty or thirty years after the exposure that contributed to it. By the time the harm is visible, the connection to a specific home or a specific radon level has long since become impossible to establish with certainty.


This absence of immediate feedback removes the primary mechanism by which most people learn to associate a cause with its effect. There is no moment at which a family living with elevated radon thinks: something is wrong, this must be the radon. The experience of radon exposure is identical to the experience of clean air, right up until the diagnosis that may come decades later — and even then, the link to radon is rarely made explicitly.


The invisible, undetectable nature of the hazard. As we discussed in our post on whether radon can be seen, smelled, or tasted, radon provides no sensory feedback of any kind. Humans are reasonably good at responding to threats they can perceive — smoke, strange odors, visual warning signs. We are considerably less good at responding consistently to threats that provide no perceptible signal. Radon asks people to take action against something they cannot observe, cannot feel, and cannot verify without specialized equipment. That cognitive demand, modest as it sounds, is a genuine barrier to action for many people.


The voluntary nature of the response. Most of the major public health successes of the past century have been driven by mandatory measures rather than voluntary ones. Seatbelt laws dramatically reduced traffic fatalities not because public awareness campaigns changed behavior but because legal requirements and enforcement did.


Smoke detector requirements in building codes ensured widespread adoption that voluntary recommendations had not achieved. Radon mitigation, by contrast, remains almost entirely voluntary in most states including Michigan. The absence of a legal mandate means the response depends entirely on individual initiative — a far less reliable mechanism for achieving population-level behavior change than regulatory requirements.


The framing challenge. Communicating risk effectively requires framing that connects with how people actually think about probability, time, and personal relevance. Radon risk involves a small-to-moderate probability of a bad outcome thirty years in the future in exchange for a modest cost today. That is a difficult risk communication challenge.


People consistently underweight small probabilities of future harms relative to certain present costs — it is a well-documented feature of human cognition rather than irrationality. Radon awareness campaigns that simply state the statistics have not succeeded in overcoming this tendency, and more psychologically sophisticated approaches to the communication challenge remain underdeveloped.


What Would Change the Picture


The states that have made the most progress on radon awareness and testing rates are generally those that have moved beyond voluntary awareness campaigns toward structural changes that encounter people at decision points rather than asking for proactive action.


Real estate disclosure requirements encounter buyers and sellers at a moment when they are already paying attention to the specific home in question and have strong motivation to act on information about it. Minnesota's comprehensive real estate disclosure framework has demonstrably increased radon testing rates compared to states without similar requirements. Indiana's seller disclosure form, which includes known radon hazards, creates a natural touchpoint for the radon conversation in every residential transaction.


Building code requirements for radon-resistant new construction in Illinois and several other states mean that hundreds of thousands of new homes are built with the infrastructure for radon control as a standard feature rather than a retrofit. This does not eliminate the problem but it normalizes radon awareness in a way that voluntary programs have not achieved.


Healthcare system integration — primary care physicians asking about home radon testing as part of routine preventive care conversations, the way they ask about smoke detectors, carbon monoxide detectors, and other home safety measures — would create hundreds of millions of touch points annually between radon information and people who are in a receptive frame of mind for health guidance. This integration has not happened at scale, but pilot programs have demonstrated its potential effectiveness.


Why Michigan Specifically


Michigan's awareness gap is particularly consequential given the state's geology. As we have established across the county-specific posts in this series, a substantial portion of West Michigan sits in EPA Zone 1 territory — the highest predicted radon potential designation. One in four Michigan homes tests above the EPA action level. Elevated radon has been detected in all 83 counties. Yet Michigan has no mandatory disclosure requirement, no building code mandate for radon-resistant construction outside of specific Zone 1 areas, and no comprehensive school testing requirement.


The combination of high geological risk and low regulatory attention means Michigan homeowners are navigating a significant health risk largely without the structural support that other states have provided. The awareness work that advocacy organizations, public health departments, and companies like All Michigan Mitigation do in this environment is genuinely substituting for regulatory infrastructure that does not yet exist.


The Simplest Response to the Awareness Gap
